I am currently working at West Midtown Medical Group (WMMG), which is
located at the corner of 35th Street and Eighth Avenue in New York, NY
(westmidtownmedicalgroup.net, 2016). They offer various treatment and medical
services for both individuals and groups either at their health center or at the
client’s preferred location. The mission at WMMG is to “improve the quality of
life of their patients by giving high quality and comprehensive medical and
treatment services” (westmidtownmedicalgroup.net, 2016). This mission is
achieved by using a mind-body healing approach to help their clients enjoy a life
that is healthy, full, and in recovery. WMMG understands that the mind and the
body function as one and must be in tune for the patient to recover. They also
believe that each person has a unique way of healing and hence customize the best
treatment approach based on an individual. Further, they embrace evidence-based
approaches to treatment and medicated assisted treatment in helping the patient
start healing from within. Therefore, they endeavor to provide high-quality
treatment in an environment that is caring, safe, and supportive.
WMMG offers a range of medical and treatment services. They offer high-quality
and compassionate primary medical care at their clinic facility from simple
physical examinations to treatment of complex cases such as cancer and
HIV/AIDS. They also offer Gynecologic screening, cancer screening, and
treatment for sexually transmitted diseases. Besides primary medical care,
WMMG also provides psychiatric services, Ambulatory opiate detoxification, and
substance abuse treatment. Ambulatory opiate detoxification allows for longer
and more individualized treatment process than opiate agonist maintenance
therapy hence is preferred at WMMG. Further, WMMG expects more success in
the program by linking patients to ongoing treatment and also counseling at the
end of the detoxification process.
Psychiatric services involve addiction treatment programs for people with alcohol
or drug addiction, which help address the psychological, sentimental, and physical
needs of the patient. Under substance abuse treatment, they focus on methadone
treatment & counseling, buprenorphine therapy, alcohol and substance abuse
services (ASAS), and medication assisted treatment using buprenorphine and
naltrexone. All these treatment programs are supervised and individualized for the
patient by a professional medical team of physicians certified by the Board.
I work as a counselor at WMMG, and I am also doing my internship there. I am
tasked with meeting with patients and counseling them, helping them overcome
their social, family, and individual issues to become better citizens. I have
performed both individual and group counseling and assessments under
supervision from the professional staff at WMMG. I talk with the clients and
spend interactive time with them, getting to know them on a personal level and it
helps me in my assessment. I have been able to use the social work approaches
and the concepts that I have learned in class during my internship. They have
helped me have a better understanding of how to help clients solve their problems.
Specifically, I have been able to practice on identifying clients’ problems, analyze
their obstacles and strengths on the three levels, formulate a case based on
gathered information and propose an intervention plan. These aspects are very
crucial to a counselor and I have been able to practice and learn them better at
WMMG.
The client under assessment was a 42 year old African American Female who was
struggling with substance abuse specifically cocaine, heroin and spathic alcohol
use. A friend referred the client to the program and she decided one day to see
what the program had to offer. The client has a history of Schizoid affective
disorder, which causes her to hear voices. She is under treatment for substance
abuse, schizoid affective disorder, and opiate methadone treatment. I was able to
gather both personal and family information from the client that enabled me to
analyze her situation better. From an individual level, the client demonstrated
insight into her mental illness and willingness to attend both individual and group
sessions. On the family, level she has support from her father and her aunt.
Furthermore, the death of her mother is greatly affecting her because she is still
going through the grieving process. On her environmental level, she’s facing a
challenging atmosphere where people are activity using and there are drug dealers
in her building.
I was able to formulate a case of the client from the above information, and my
analysis revealed that her biggest challenge was her defeated mentality as well as
her inability to say “NO”. In my intervention plan, I focused more on a
collaborative effort to help the client in her ASAS and mental health treatment
program. I also saw the need to refer the client to CCM for her mental health
issues. The other important step would be refer her to a higher level of care to
help her with her drug use and to refer her to a therapist to help with the feelings
of grieve over the loss of her mother.
The cause of action would be to meet with the patient two times a month for
individual sessions to discuss toxicology reports and treatment plan goals. She
will participate in group therapy, which consist of relapse prevention, emotional
awareness and motivational enhancement. I will be monitoring her toxicology and
urine samples on a weekly basis. I will also collaborate with her ASAS counselor
and mental health provider periodically to follow up with her progress.
